What slips / information you need to process your tax return

  • Last year’s tax return and Notice of Assessment
  • Social Insurance Number for you and your spouse/common-law partner.
  • List of dependents & exact birth dates
  • All receipts of income such as:
    • Salary or wages (T4 Slip)
    • Pension, retirement, annuity and other income (T4A)
    • Statement of Employment Insurance & other Benefits (T4E)
    • Registered Income Fund Income (T4RIF)
    • Registered Retirement Savings Plan Income (T4RSP)
    • Statements of trust income (T3)
    • Canada Savings Bond Interest Benefits
    • Canada Pension Plan Benefits (T4AP)
    • Old Age Security Benefits (T4AOAS)
    • Interest and Dividend (T5)
    • Labour sponsored fund tax credit (T5006)
    • Social Assistance (T5007)
    • Worker’s Compensation (T5007)
    • Statement of Securities Transactions (T5008)
    • Employment Insurance (T4E)
  • All receipts for expenses, for example:
    • Child Care Expenses (plus caregiver’s Social Insurance Number)
    • Medical Expenses
    • Interest Expenses
    • Adoption Expenses
    • Charitable Donations
    • Property taxes or rent paid for tax year
    • Monthly Transit Passes and Receipts
    • Political Donations
    • Union Dues
    • Tuition fees/Education Amount (T2202/T2202A)
    • Student Loan Interest
    • RRSP Contributions
    • Disability Certificate (if first year of claim or previous one has expired-T2201)

If claiming tax credits for non-resident spouse or children, dates of birth, complete addresses and proof of payment (generally receipts for money orders) for funds sent to dependents.
